We have a Problem Users forum, a Trash Bin forum and a set of contact templates. All posts removed from the community are placed in the Trash Bin forum. We do not delete (permenantly) any posts. This allows for better recordkeeping and easier mistake correction.
All violations are then logged in Problem Users, where each user gets their own thread with their username as the title. A link to the violation in the Trash Bin forum, the violation and the action taken. For instance:

Inflammatory. PM sent.
Then the user is sent a private message using our contact templates, which are a list of pre created messages that have blanks that are filled in/that can be adjusted to meet your needs. _________________ Patrick O'Keefe - CommunityAdmins.com Administrator

I find this one of the most sensitive issues on my board. Pulling up an erring member can be hard for an admin to do especially when the member has been a regular contributor to your forum. Luckily I've rarely had to do this, but I've felt the pinch when having to "moderate". It's tough...
If you don't take any action, it will probably come across as favouritism. On the other hand, if you take action, you might end up losing a valuable member...
It all comes down to how you approach your community. I always consider my members to be more important... issues can always be resolved as and when they arise, but a good, active member is worth his weight in gold. I always use PM if I have any suggestions to make and usually do so in a manner which comes across as understanding and sympathetic.
Only if a member tried my patience beyond endurance, I would take a very firm stance... _________________ Literary Forums

My system is almost identical to Patrick's, much of which were inspired by his setup. It's a great way to do things and helps you keep a nice, clean forum with a paper trail should you need it. The violations themselves become a training manual because moderators can browse the trash bin and see what was deleted and why. _________________ Chrispian H. Burks

We have a trash forum, usually only spam goes in there though.
As we don't have guideline's, we don't really have any issues.
People soon learn how the site works and what is expected of them, and if they still come in being an idiot, the community will ... "let them know" they are dis-liked. Usually they leave, but 2 (out of near 3,000) member's have been banned for continually coming back and being pathetic.
Most leave of their own accord before an admin or mod steps in. _________________ Marc
