I've had to demote and ban people I once considered friends. But, many times that is when they show their true colors.
But, as far as them just being inactive, etc. most of them have understood the demotion. I don't know that I've lost friends in that scenario. After a period of time has passed with an inactive staff member, I generally contact them first to see how it is going and how they are doing, saying that I'm looking forward to getting them back without mentioning leaving, etc., hoping to hear back from them. If they don't reply, I have to remove them. Many times, they have taken this opportunity to leave/offer to leave. If they aren't able to come back in a reasonable period of time, then I have to let them go. Other times, they'll leave it up to me. If someone has really been my friend, I don't lose them as a friend. If they haven't, then I will.
