Most bans at my community are permanent once they get to the banning stage. I have made one exception before, and thats because the person showed real change after that.
Back to the moderators and how they should be ^_<

Thats basically what i was getting at in so many words ^_^ My mods have such great perspective that they ususally dont fall into the emotional traps that others set for them. Moderators that act like machines may be terribly efficient, but if theres no human side to them, then their moderation is nothing better than a random word filter >_> gotta have some emotion behind that moderation.
